Our Service Area:
Who May Be Eligible:

Any individual at least 18 years of age who:

  • Has a medical condition that requires treatment or rehabilitative services prescribed by a physician; 

  • Has a physical and/or mental impairment(s) that handicaps activities of daily, but not so serious as to requiring 24 hours institutional care 

  • Has a reasonable expectation that preventive services will maintain or improve the present level of functioning 

  • A high chance of further deterioration and probably institutionalization of ADHC/CBAS services were not available. (For additional eligibility information, please contact the office for more details).

Program Cost:

Adult Day Health Care/Community Base Services (CBAS) are available at no cost to eligible Medi-Cal beneficiaries enrolled with Medi-Cal Managed Care Plan (i.e. LA Care, Health Net, Kaiser, Molina, etc.) Private Pay plans are also available on a sliding scale.

Eligible veterans may also participate through the USVA Rehabilitation Assistance Program. 

LA County cities of Long Beach, Carson, Compton, Paramount, Bellflower, Lynwood, Lakewood, Norwalk, Gardena, Torrance, Lomita, and the unincorporated areas of Harbor-Gateway, Rancho Dominguez and Wilmington San Pedro areas of the City of Los Angeles.
